How It Works
1. Eligibility & Waiver Enrollment: IHCS is provided under ODP’s 1915(c) waivers (e.g., Consolidated, Community Living, and Person/Family Directed Support). These waiver options allow Medicaid funds to be used for services at home and in the community.
2. Person-Centered Planning: Supports are defined in an Individual Support Plan (ISP), developed annually through a team process that identifies personal goals, skills need, and desired community outcomes.
3. Flexible Implementation: Supports Brokers, Supports Coordinators, and providers work together to hire, train, and match staff based on the individual's preferences and everyday routines.
